Criteria, checks, and next step

01

Current presentation

Clarify what is happening now.

  • What is new, different, or worsening?
  • Which associated signs affect the level of concern?
02

Existing plan

Review professional guidance already received.

  • What did the clinician advise about recurrence or change?
  • Does the client have a written emergency or follow-up plan?
03

Scope boundary

Separate therapist tasks from medical tasks.

  • Which questions belong to a licensed medical professional?
  • What emotional or behavioral support is appropriate today?
04

Communication

Prepare a concise next step.

  • What information should the client bring to care?
  • Who will be contacted, and under what conditions?
Summary

Reduce the decision to a few points

  1. Document the referral rationale neutrally.
  2. Avoid reassurance that exceeds available evidence.
  3. Review local legal and ethical requirements.
